We are now accepting NEW PATIENTS. Make an appointment by calling 919-833-3111.  We accept all major insurances, but if you are uninsured we offer a Discount Fee Program. Please download an application and follow the directions for  Making An Appointment.

View Map. Take I-40 towards Raleigh to Rock Quarry Road (exit 300). Make a left at the end of the exit ramp. At the second stop light, veer to the left on to Rock Quarry Road. Rock Quarry Road Family Medicine is the first building on the right.

Horizon Healthcare for the Homeless Program

Horizon Healthcare for the Homeless program provides primary care and support services to people experiencing homelessness. We have expanded our program, and patients are seen at our Rock Quarry Road site as well as our other practice sites in Wake and Franklin Counties. Services include primary care, behavioral health and substance abuse counseling, smoking cessation, nutritional counseling and diabetes education.  Our homeless patients receive the same quality care by board-certified providers that all of our patients see. We have full-time licensed clinical social workers and licensed clinical addiction specialists.

Men, women, and children are welcome. We have many partners in helping the homeless: the Wake County Continuum of Care Collaboration, the Wake County Homeless Working Group, the Wake County Housing and Homeless Coalition, and the North Carolina Coalition to End Homelessness.

Case Management, Nutrition Counseling and Diabetes Education, Diagnostic Tests/Screens, Drug Assistance Program, Behavioral Health Counseling, Medicare/Medicaid Eligibility Assistance, Social Workers, Dental care.

Providers

Anthony Robinson, PA

 Anthony received a Bachelor of Science degree in Biology from the University of North Carolina (Chapel Hill, NC) in 2004. Anthony received his Master of Health Sciences degree and Physician Assistant Certificate from the Duke University Physician Assistant Program (Durham, NC) in 2007.

Brooke Findley, RD

 

Brooke joined WHSI in August 2012. She graduated from the University of Florida with a Bachelor of Science in Food Science and Human Nutrition in 2002. She received her Master of Science in Food Science and Human Nutrition degree from the University of Florida in 2007. She is happy to be a part of the Wake Health Service team and enjoys helping others achieve better health through nutrition.

Sara Johnston, PA

Sara has practiced in tthe areas  of  Internal Medicine and Cardiology/Cardiothoracic Surgery.  She is a graduate of  Wake Forest University with a Master of Medical Science Degree in Physician Assistant Studies. 

Catherine Thrash, LCSW, LCAS

Catherine joined Wake Health Services in August, 2004. She received her Masters degree in Accounting from Mississippi State University (Starkville, MS) in 1984 and her Master of Social Work degree from Delaware State University (Dover, DE) in 1999. She is a Licensed Clinical Social Worker and Licensed Clinical Addictions Specialist and has been an advocate for our homeless population for eight years.

McTisa Lane, FNP

McTisa Lane is a graduate, Magna Cum Laude, from Winston-Salem State University with a  Master of Science in Nursing: Family Nurse Practitioner.

Shannon Wallace, LCSW, LCASA

Shannon Wallace, MSW, LCSW, received her Master of Social Work in 2008 from the University of North Carolina at Chapel Hill in 2008 and earned a Certificate in Substance Abuse Studies in 2008.  She completed her undergraduate work at Meredith College in 2004. Shannon recently worked at the NC Department of Health and Human Services UNC STEP Research Clinic at the North Carolina Psychiatric Research Center.  She is a Licensed Clinical Social Worker and Clinical Addictions Specialist-Associate.

Doris Nicolas-Mir, RD

 Prior to joining Wake Health, Doris worked at the Aramark-Johnston Medical Center as their Clinical Dietitian. She received her undergraduate degree from Florida International University, and her Masters of Public Health from the University of Miami.   She is fluent in Spanish, as well as English.
